Overview

Presbyterian Homes & Services is seeking an Hospice Liaison for its team.

The Hospice Liaison advances PHS’ mission by building trusted relationships and providing non-clinical hospice education to PHS senior living communities, residents, families, and referral partners. Through participation in community transition and at-risk huddles, proactive outreach, and collaboration with community and clinical leaders, this field-based role supports earlier identification of residents who may benefit from hospice and helps ensure timely, informed decisions that respect each individual’s freedom of choice. The Hospice Liaison reports to the Director of Business Development.

Responsibilities

The Hospice Liaison serves as the field-based, non-clinical education and relationship resource for assigned PHS senior living communities and referral sources, working collaboratively with Campus Administrators, Clinical Administrators, Genevive primary care, Optage Home Health, hospice clinical leadership, and the centralized Access and Admissions team.

Key Responsibilities included but are not limited to:

  • Support earlier recognition of residents who may benefit from a hospice evaluation and facilitate timely, informed consideration of hospice services in a manner consistent with patient freedom of choice.
  • Participate in weekly community transition and at-risk huddles with Genevive primary care, Optage Home Health, and community leaders to recognize potential hospice needs, promote timely discussion of available services, and coordinate warm handoffs to the centralized Access and Admissions team.
  • Build and maintain collaborative relationships with Campus Administrators, Clinical Administrators, and other key stakeholders; monitor referral trends, identify opportunities for education or follow-up, and conduct proactive outreach in accordance with established partnership standards.
  • Provide accurate, mission-first, non-clinical education to residents, families, and referral sources regarding the hospice benefit and Optage services; refer clinical questions to the registered nurse team and direct eligibility and admission decisions through Access and Admissions and the appropriate hospice physician.
  • Conduct all education, outreach, and relationship-building activities in a manner that supports informed decision-making, respects each individual’s freedom of choice, and complies with applicable anti-steering requirements and organizational policies.
  • Document outreach activities, huddle participation, referral trends, identified barriers, and follow-up actions in the designated system, and report on a regular cadence in accordance with established reporting expectations.
  • Develop and deliver scheduled hospice education for community leaders, team members, residents, families, and referral partners using approved materials, adapting content to the needs of each audience.
  • Partner with Access and Admissions and community teams to support timely follow-up on referrals, identify barriers or service gaps, and facilitate appropriate handoffs without participating in clinical assessments or eligibility decisions.
  • Maintain a consistent presence within assigned communities and develop outreach plans based on community needs, referral trends, and opportunities for education and relationship development.
  • Gather feedback from communities, residents, families, and referral partners; identify recurring questions, barriers, or service needs; and recommend improvements to education, communication, referral processes, and the overall customer experience.

Qualifications

  • Bachelor’s degree in healthcare, communications, business, or a related field preferred; equivalent experience in hospice, home care, senior living, or healthcare community relations is accepted in lieu of a degree.
  • Two or more years of experience in hospice, home health, senior living, or healthcare outreach, education, or community relations preferred.
  • Working knowledge of the Medicare hospice benefit and the hospice philosophy of care, including patient rights and informed decision making, with the ability to describe services accurately without providing clinical advice or steering patient choice.
  • Valid driver’s license, reliable transportation, and the ability to travel to PHS communities and referral sources across the assigned market (Minnesota or Eastern Wisconsin).
  • Excellent interpersonal, communication, and organizational skills.
  • Proficiency with Microsoft Office and other common computer programs.
  • Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.
  • Demonstrated compatibility with PHS’ mission and operating philosophies.
