Optage, the Home & Community division of PHS is seeking a full-time medical administrative professional to fill our Scheduling Coordinator position.  The Scheduling Coordinator is responsible for coordinating schedules with clients, families, caregivers, and employees
to ensure service delivery per the established plan of care.

REPORTING STRUCTURE
The Scheduling Coordinator reports to the Optage Services Manager or management designee.

 

Responsibilities

Duties include but are not limited to;

 

Scheduling and Coordination:

  • Create and maintain schedules for home health visits, matching client needs with caregiver availability and qualifications.
  • Communicate changes or updates in schedules promptly to clients, families, caregivers, and employees.
  • Monitor client care needs and adjust schedules as necessary to accommodate changes in condition or service requirements.

Communication and Customer Experience

  • Facilitate communication among patients, caregivers, employees, and interdisciplinary teams to ensure seamless care delivery.
  • Provide timely follow-up on client inquiries, requests, and feedback.

Medical Records

  • Maintain client records and electronic files to ensure information is accurate and complete. Commit to the highest, feasible level of confidentiality possible ensuring adherence to all HIPAA security requirements. Assist in quality assurance activities.

Qualifications

  • High school diploma or equivalent.
  • Minimum of one year experience in home health, hospice, senior housing, or health care-related environment
    preferred.
  • Must understand basic medical/clinical terminology and reimbursement.
  • Must have strong communication, problem-solving skills, and customer experience focus.
  • Must be able to work independently and multi-task with minimal supervision.
  • Must be a team player, willing to take directions and accept responsibility for specified objectives.
  • Demonstrated computer proficiency including Microsoft Office and other computer programs.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all
    customers.

Pay Range

The estimated starting pay range for this role is $20.22 - $24.92 per hour. This good faith estimate is based upon PHS’s typical starting pay range for this role. Actual pay range depends upon many factors including each individual employee’s qualifications and market conditions and is subject to change.

Full-time benefits-eligible employment begins at a minimum of 30 hours per week:
