Optage, a division of Presbyterian Homes and Services, is seeking a clinical professional to join its Home-office team as the Clinical Intake Coordinator to support Home Health with intake services.

REPORTING STRUCTURE
The Optage Clinical Intake Coordinator reports to the Optage Services Manager or designated leadership team member.
The Clinical Intake Coordinator is responsible for the clinical review and coordination of the client intake process for service lines, including Home Health, Hospice, and Rehabilitation. The Clinical Intake Coordinator is responsible for maintaining positive relationships with customers and referral sources and ensuring all pertinent information is obtained and processed. This role is a clinical liaison to referral sources, providing education, evaluation, and coordination of services.
