Avalon Square/Stair Crest is seeking a Temporary Physical Therapist to join our team, working full time hours through the duration of this assignment This temporary position provides critical client care during employee medical leave.  Tentative start date is the middle of March, for an approixmate 20 week duration, this assignment will end when employee returns from leave.  While this is a temporary assignment, there is an opportunity to be considered for a regular/on-call position with PHS upon the conclusion of this term.   

 

SCHEDULE: 30 - 40 hours a week, 5 days a week.

Responsibilities

As a Physical Therapist you will be responsible to:  

  • Conduct a comprehensive assessment & evaluation using subjective and objective measures. 
  • Develop treatment plans and programs in accordance with physician orders.  
  • Provide quality treatments, actively progressing goals to achieve highest quality of life.  
  • Conduct screens and provide education to residents/clients, families, caregivers, and staff.  
  • Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.  
  • Provide accurate and timely documentation & billing in electronic medical record.  
  • Demonstrate understanding and knowledge of payer sources and regulatory requirements.  
  • Complete and document supervisory visits per regulations.  
  • Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.   

Qualifications

  • Graduate from an accredited Physical Therapy program.
  • Current licensure in the state(s) of practice as required.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
