We are seeking a Director of Integrated Home Health & Rehabilitative Services to provide strategic and operational leadership for PHS’s home health and rehabilitative programs. This role is responsible for ensuring the effective integration, coordination, and performance of post-acute care services across the continuum.

 

Responsibilities

The Director of Integrated Home Health & Rehabilitative Services is responsible for providing support to Presbyterian Homes & Services home health and rehabilitative programs. This position holds responsibility for the strategic integration and operations of the post-acute care services throughout the continuum. The Director of Integrated Home Health & Rehabilitative Services works collaboratively with Operations, Home and Community Based Services, and Campus Leadership teams to provide high quality services to residents and customers.

 

Responsibilities include, but are not limited to:

  • Manage and oversee the integration of home health and rehabilitative services delivery models and work continuously to improve current systems.
  • Identify the needs of home health and rehabilitative services in the organization. Assure an effective communication and problem-solving process is in place to optimize the flow of information between site and PHS Management & Services. Assure collaboration to achieve desired organizational outcomes and site goals.
  • Develop new systems and standardization of home health and rehabilitative services policies, procedures and practices across the organization that integrate population health and value-based care philosophies and are consistent with regulations and established best practices.
  • Use quality management and lean processes to evaluate effectiveness of home health and rehabilitative services processes.
  • Assist in strategic and financial planning to develop appropriate revenue and expense assumptions for annual budgets.
  • Serve as a point person for related software initiatives.
  • Set and measure performance/outcome standards for all areas of home health and rehabilitative services functions across the continuum, including staffing, policies, procedures, forms, etc.
  • Coordinate services with other health professionals inside and outside PHS.
  • Promote a spirit of cooperation with all regulatory organizations managing with an expectation of appropriate compliance and corrective measures.
  • Integrate the PHS Christian Culture; reinforce the Christian heritage and culture through communication and education systems; with all customers recognize the importance of spiritual matters to clients, families, and employees.
  • Establish and maintain effective working relationships with outside agencies and organizations to establish effective benchmarking opportunities and participate with the various community constituencies to achieve mutually beneficial results.
  • Create and maintain a work environment for employees that is conducive to high productivity and morale. Establish and maintain effective communication systems with employees.
  • Maintain knowledge of organization and industry policies and practices.

Qualifications

MINIMUM QUALIFICATION 

  • Graduate of an accredited PT, OT, SLP program with current license in state(s) served. Master’s degree preferred.
  • A minimum of five (5) years of leadership experience in rehabilitative services.
  • Demonstrated ability to write and implement policies and procedures.
  • Knowledge of State and Federal regulatory requirements for post-acute services. Working knowledge of regulatory compliance in this field.
  • Knowledge of reimbursement under Medicare, Medicare Advantage, and commercial payers.
  • Excellent written, interpersonal, and communication skills; adaptability, flexibility, and creativity.
  • Ability to travel to PHS sites and work a flexible schedule to support site and organization needs.
  • Excellent interpersonal, communication, and organizational skills.
  • Proficiency with Microsoft Office and other common computer programs.
  • Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.
  • Demonstrated compatibility with PHS’ mission and operating philosophies.

Pay Range

The estimated starting pay range for this role is $134,000 -167,398 per year. This good faith estimate is based upon PHS’s typical starting pay range for this role. Actual pay range depends on many factors including each individual employee’s qualifications and market conditions and is subject to change.

Full-time benefits-eligible employment begins at a minimum of 30 hours per week:
