Overview

Presbyterian Homes and Service is seeking a full time Home Care Director to lead its Optage Home and Community Based Services team.

 

The Home Care Director assures the development and effective integration of the organization’s services within the context of an overall health delivery system. The Director will perpetuate Optage Home Care as a financially sound, high-quality Medicare certified agency. The Director gives direction and leadership to the achievement of annual goals and objectives and assures there is alignment with the organization’s established mission and values.  

 

This position is a key member of PHS leadership/management team, responsible for acknowledging the Christian purposes of PHS, reinforcing the Christian heritage, and establishing a culture of caring consistent with the teachings of Jesus Christ.

Responsibilities

The Director gives direction and leadership to the achievement of annual goals and objectives, and assures there is alignment with the organization’s established mission and values.  In addition, they will also partner closely with PHS Rehabilitation (Outpatient) Therapy to offer an integrated therapy program across the PHS continuum.

 

The Home Care Director will:

  • Integrate the PHS Christian Culture; reinforce the Christian heritage and culture through communication and education systems; with all customers recognize the importance of spiritual matters to clients, families, and employees.
  • Ensure the effective operation and integration of existing programs, i.e. skilled visits and private duty with an emphasis on continuous quality improvement, customer service, health care outcomes, human resource development, staff supervision, and financial contributions.
  • Achieve or exceed financial and operational goals by managing resource allocations and directing the annual planning process (revenue, expenses, and capital) approved plans. Provide leadership in the development of annual strategic and financial plans—give input to the setting of targets and assumptions; assure plans are well thought out and cover all needed items; set expectations for quality improvement.
  • Create and provide leadership for an effective management team and its programs including the development of management talent, effective organizational structures, and operating procedures.
  • Provide positive leadership in influencing regulatory organizations while promoting a spirit of cooperation. Manage program to achieve appropriate compliance and corrective measures to meet regulatory requirements.
  • Partner with PHS Clinical, Regional Operations, Therapy and other functions to develop a product that integrates services, optimizes transitions through PHS continuum (TCU, Homecare, Outpatient Therapy) and prioritizes customer experience.
  • Implement Lean management techniques into the Home Care environment.
  • Represent the interests of PHS with trade associations, legislative processes, community, and churches. Representation is consistent with the goals and strategies of PHS in all public and professional endeavors.
  • Manage complaints or concerns from clients, families, employees, and/or other site customers.
  • Promote continuous self development and growth. Exhibit technical competence in the field by being familiar with and ensuring compliance with regulations and established best practices. Maintain current knowledge of community resources and organization and industry policies and practices. Use discretion and independent judgment to compare and evaluate possible courses of action and decision-making.
  • Other areas as identified to support the aligned goal setting and achievement.

Qualifications

  • Bachelor’s degree or higher in health care preferred.

  • A minimum of five (5) years experience in health care leadership preferably in a Medicare certified Homecare program , Assisted Living or Care Center. .
  • Demonstrated competencies in health care management and operations, strategic planning, quality management, client and family support systems, and community outreach.
  • Excellent written, interpersonal, and communication skills. Excellent detail orientation and organizational skills.
  • Demonstrated P&L management expertise and skill.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
  • Ability to provide transportation in the metro area.
