Overview

Presbyterian Homes & Services is seeking a full time Rehabilitation Program Manager to join its Rehab team at GracePointe Crossings in Cambridge, MN.

This is a large campus serving residents in long term care, transitional care, and those needing outpatient services.  

 

This position requires experience in leadership & expertise in rehabilitation serving older adults across the continumm of care. The preferred candidate would be a licensed physical thearpist but other licensed OTs or SLPs are eligible to apply.

Responsibilities

The Rehabilitation Program Manager is responsible for planning, organizing, developing, evaluating and effectively directing the rehabilitation staff and program in a transitional care setting in order to ensure the delivery of high quality resident care and services consistent with regulations and established best practices.

 

The Rehabilitation Program Manager has responsibility as a member of the site leadership team to participate in the planning and implementation of strategies and programming that assure high quality resident care while achieving short and long-term operational goals and objectives for the site.

Qualifications

  • Graduate of an accredited program.  Registered therapist with current licensure in state(s) for assigned program oversight.
  • Three to five (3 - 5) years leadership experience in rehabilitation services.
  • Demonstrated working knowledge of the interrelationships of speech, occupational, physical therapy, or other rehabilitation/wellness programs in patient care.
  • Demonstrated working knowledge of Medicare and third party reimbursement and qualification.
  • Ability to work cooperatively with other health care providers, staff, family and residents in meeting resident/patient needs.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
