    Presbyterian Homes & Services - Optage Home & Community Based Services program is seeking a Home Care Registered Nurse to join its team to serve clients. 

    1. A Full Time Home Care RN for the South Region. 
      • The sites you would provide care in are: McKenna Crossing in Prior Lake, MN; Founders Ridge in Bloomington, MN; PHS of Bloomington in Bloomington, MN and Orchard Path in Apple Valley, MN.

    This opening is a Weekday Day position.


    This role offers flexibility. You are able to organize your calendar by scheduling your visits and have the ability to document at home. 


    The Registered Nurse (RN) is responsible for the provision and monitoring of high quality clinical care and services to residents consistent with regulations and established clinical best practices. 


    • Registered nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
    • Interest in gerontological nursing, rehabilitative and restorative clinical practices.
    • Demonstrated compatibility with PHS's mission and operating philosophies.
    • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
