    Presbyterian Homes & Services is seeking a Registered Nurse to join our ResoLute grant program as a Quality in Living Specialist (QUILS). ResoLute stands for: “Resident Empowered Solutions on Living Until the End”.  The QUILS will support the resident and their loved ones embrace aging with purpose and determination by upholding what is most important to them. The QILS, through conversation and involvement with all stakeholders, will be the catalyst for improving late life living with attention to the work of aging. ResoLute is an upstream palliative approach that begins at the onset of the late life stage. In this role you will facilitate conversations and collaborative relationships between residents, their loved ones, the care team, and other providers surrounding the residents’s late life care plan and what matters most to them.    

     

    This position will play a key role in the development and implementation of the Empira ResoLute Program that seeks to challenge the current status quo approach to late life living and dying, strengthen understanding of the importance of truthful prognostication, and inspire individuals to embrace the work of aging. 

     

    This opening is at our Johanna Shores location in Arden Hills and is full time, 40 hours a week. 

    Responsibilities

    1. Facilitate “work of aging” care planning with a balanced approach to biological, psychological, and social late life needs of residents.
    2. Develop a trusting relationship with all stakeholders.
    3. Assess and discover knowledge gaps of resident and family understanding of diagnosis, disease trajectory, and individual prognostication
    4. Unite resident, family, providers and care team in understanding of the resident condition.
    5. Identify and communicate priorities, hopes, fears, life reflections and legacy wishes of the resident.
    6. Create a late life plan jointly with the resident, provider, care team and their loved ones based on what matters most to them

    Qualifications

    • Current RN licensure in the State of Minnesota.
    • Current Long Term Care or geriatric clinical experience preferred.
    • Proven passion, confidence, and candor required for late life conversations.
    • Ability to interact with all stakeholders from varying social, cultural, and educational backgrounds.
    • Must be able to function independently, demonstrate flexibility and have the ability to handle and prioritize multiple tasks simultaneously.
    • Ability to identify and resolve exceptions and to interpret data.
    • Desire for continuous growth and development.
    • Must be willing to travel locally as needed, up to 25% of the time. (no overnight stays).
    • Flexibility in working hours to accommodate all stakeholders involved in late life conversations (some will occur in the evening or on weekends).
    • Excellent oral, written, and interpersonal communication skills.
    • Demonstrated computer proficiency including Microsoft Office and other common computer programs.
    • Must be a team player, willing to take direction and accept responsibility for meeting specified objectives.
    • Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.
    • Demonstrated compatibility with PHS’ mission and operating philosophies as well as the ResoLute Program.
