Presbyterian Homes & Services, Optage Home and Community Based Program is seeking a full time Regional Case Manager to join its team.
The Regional Case Manager is responsible for implementing, evaluating and effectively directing the staff and systems of PHS’ Home and Community Based Optage Home Care program for assigned region in order to ensure the delivery of high quality client care and services consistent with regulations and established clinical best practices.
The Regional Case Manager has responsibility as a member of the home care leadership team to participate in the planning and implementation of strategies and programming that assure high quality client care while achieving short and long-term operational goals and objectives for the region.
