Interlude Transitional Care is seeking a full-time Director of Nursing / Clinical Administrator to join its team.

 

Interlude Transitional Care is a collaboration between Presbyterian Homes & Services (PHS) and Allina Health Care.  Interlude is located on the campus of Allina Unity Hospital in Fridley MN.  This transitional care center provides post-acute care patients a clinically advanced yet soothing environment as they transition from hospital to home. This next-generation transitional care center is focused on hospitality, wellness and service, with 55 private suites.

Responsibilities

The  Clinical Administrator is responsible for overseeing the Interlude Transitional Care clinical staff and program.   The position utilizes skills in planning, organizing, developing, evaluating and effectively directing the clinical staff and program in order to ensure the ongoing delivery of high quality resident care and services consistent with regulations and PHS standards.

 

The  Clinical Administrator has responsibility as member of the Interlude leadership team to participate in the planning and implementation of strategies and programming that assure high quality resident care while achieving short term operational goals and objectives for the site.

Qualifications

  • Registered nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
  • 3-5 years experience as a Director of Nursing/Clinical Administrator in transitional care.
  • Demonstrated competencies in health care management and operations, RAI processes, quality management, staff development, resident and family support systems, and community outreach.
  • Demonstrated knowledge and experience in gerontological nursing, rehabilitative and restorative clinical practices.
  • Demonstrated computer proficiency in Microsoft Office applications and electronic medical records.
  • Knowledge of State and Federal regulatory requirements for long-term care.  Demonstrated successful working knowledge of regulatory compliance. 
  • Excellent written, interpersonal, and communication skills; adaptability, flexibility, and creativity.  Ability to communicate with a variety of people.
  • Ability and willingness to travel as needed to Wisconsin, Iowa and other PHS facilities.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
