The Clinical Coordinator – Primary Care is responsible for providing leadership and support for the work of primary care staff (RN/LPN, Social Worker) to ensure the delivery of high quality primary care and services consistent with regulations and established best practices. The Clinical Coordinator works collaboratively with the Chief Medical Officer, NPs and other Optage Primary Care, Home & Community Based Services, Hospice and Senior Dining staff, Presbyterian Homes' Site staff, and functional directors of assigned areas of accountability.
